Course Description:

The First Steps ACP Facilitator Certification course is intended for healthcare providers (nurses, all levels of social workers and physicians), clergy, and others involved in providing ACP facilitation. The purpose of this program is to assist participants in learning effective communication and interview skills for person-centered ACP facilitation. Prerequisite for this course is an online facilitator course component, Respecting Choices Online Facilitator Course. Pre-Course assignment: Review state-specific Advance Directive (AD)/Durable Power of Attorney for healthcare (DPOA) forms and AD form guidelines, personal discussion exercise, and completion of a DPOA to bring to course.
