Course Description:

This presentation reviews key aspects of the team-based care approach to care delivery in the primary care setting that involves a multi-disciplinary approach. The training reviews the historical aspects of ambulatory care, the care management process (including the Patient Centered Medical Home model, patient engagement, and health literacy), the importance of quality and data metrics, and the value of interdisciplinary communication and application to delivering effective patient-centered care.
