Course Description:

As a chronic condition, inflammatory bowel disease can seriously disrupt a patient’s quality of life. This module details how case managers can help patients engage in, and affect, their own care. It explores how shared decision making can help patients engage in the process, while looking at how healthcare providers can help the process by being more attuned to a patient’s psychosocial needs. Next, the module explores how case managers can help boost a patient’s self-care (in part by identifying possible obstacles to self-care). Finally, the module looks at the whole trans-disciplinary team, beyond the gastroenterologists and case managers, who can collaborate in this process, from nutritionists to home health workers and even family therapists.
